Human Intervention Motivation Study (HIMS) represents an industry-specific framework dedicated to coordinating substance abuse identification, rehabilitation, and medically monitored return-to-duty pathways for commercial aviators. Regulatory guidelines distinguish between utilizing HIMS-designated medical professionals and formal enrollment in a HIMS program; the latter strictly applies to airmen actively employed by an air carrier with an established HIMS infrastructure. Although independent airmen working with HIMS-credentialed providers are not in a formal employer monitoring program, the term "HIMS" is broadly applied throughout the aviation medical community to encompass these services for all applicants.

HIMS AME Services include:

  • HIMS AME Consultation: A comprehensive consultation for airmen who have been directed by the FAA to consult a HIMS-designated Aviation Medical Examiner. This initial advisory service does not require a MedXpress application.

  • HIMS Sponsorship & Monitoring: Ongoing monitoring and case management required for FAA Special Issuance maintenance. A MedXpress application may be required depending on the visit type. Please note: If a routine FAA medical examination is conducted during the same visit, it will be billed as a separate service.


HIMS/FAA Psychiatric Evaluation:
A comprehensive psychiatric assessment may be mandated by the FAA for airmen or applicants presenting with a history of psychiatric, neurodevelopmental, or substance use conditions. These evaluations are distinct from routine psychiatric care; the evaluating physician serves as an independent consultant providing an objective, third-party diagnostic opinion. The assessment process comprises a detailed clinical interview, a thorough audit of historical medical records, and, where appropriate, corroborative accounts from secondary sources such as family members, peers, or prior treating clinicians.
